Breast Surgery

One of the most common cosmetic procedures performed in the U.S. today is breast augmentation or breast enhancement. It can greatly change a woman’s breasts that she may have considered too small, or add volume to those who have lost fullness after having children.
A breast lift / mastopexy can lift and shape breasts which is often done along with breast augmentation.
Finally, a breast reduction / breast reduction surgery can correct large, sagging breasts are often the cause of neck and shoulder pain.

Procedure

Breast Augmentation

Transumbilical Approach

The incision for the breast enlargement procedure is done through the belly button / navel. By using this method, no cuts are made in or around the breast tissue. A thin instrument is inserted through this incision, passing under the abdominal skin to a position behind the pectoralis major muscle. A pocket is then created by inserting a tissue expander and filled with air. The size of the bag is checked and then complete dissection along the original incision. Finally, The saline implant is inserted and then filled with desired amount of sterile saline.

The advantages of this technique versus others: shorter surgery, less trauma and bleeding, a small incision and a faster recovery!

 

Axillary Approach

This breast augmentation incision is made in the armpit crease, reducing the display of a scar. A space is then made under the pectoral muscle, using this small incision is placed a saline implant.
For silicone implants using the new ‘Keller Funnel’ (Keller Medical Inc.) which allows the introduction of silicone implants through a small incision quickly and without rubbing against the implant tissues thereby avoiding the possible “contamination” of the implants.

Sub-mammary

The incision for this breast enlargement procedure is done under the breast in a strategic position. A pocket is then opened underneath the chest wall muscle and the implant placed in that area. A few small sutures complete the procedure.

Areolar Approach

The incision for this breast enlargement procedure is done  in a small area of the nipple / areola complex.  A pocket is then opened underneath the chest wall muscle and the implant placed in that area. A few small sutures complete the procedure.

Recovery after breast augmentation surgery

A special sports bra is placed immediately after surgery, and then used for the next few weeks. Activity is restricted and the bed rest is recommended for the first few days. Physical activity can be resumed within 4 to 6 weeks. Any discomfort can be controlled with oral medications prescribed by Dr. Spera. Absorbable sutures are used for the procedure, so removal is not required.

Are implants safe?

To date there has been no scientific evidence that implants are unsafe, cause cancer of the breast, or any other disease. Although it is not advisable to protect the aesthetic result, most women can breast feed after augmentation surgery.

Breast lift:

Depending on the sagginess of the breasts, the procedure may require different incisions:
1. Around the areola (minimal ptosis)
2. Lollipop (around the areola and vertically)
3. Inverted Anchor (around the areola, vertical and horizontal)

Working through the incisions, excess skin is removed from the bottom portion of the breast. Breast tissue is displaced in a higher position and the nipple placed in the keyhole incision.
Many of the patients undergoing breast lift also want freshness / volume to the look of the breasts, so that the choice of implants also add in the time of surgery.

Breast Reduction:

In the routine reductions, the incision follows around of the areola, continues downward and extends along the crease underneath the breast. The excess tissue is removed and the nipple / areola complex elevated to his new position. In most cases, unless your breasts are very large, the nipples remain attached. Thereby, a loss of sensitivity is avoided.

Breast liposuction

This procedure allows the surgeon to remove fat, not glandular tissue, and thereby reducing the volume of the chest by a few cup sizes.

What is the recovery of a breast lift or reduction like?

Avoid strenuous activity for 4 weeks. It may be possible to return to work in one or two weeks. Again, Dr. Spera uses dissolvable stitches. Bruising and swelling may take up to six weeks to clear, and may take a year or two for the scars to flatten and fade.
Patients undergoing breast liposuction are encouraged to resume normal activities as soon as you feel up to it. The results of breast reduction surgery are permanent, but breast size may change due to weight gain or pregnancy.
